Sensory Processing Disorder: An Interview with an Expert

Success Through Play™ Magazine interviewed leading Sensory Processing Disorder (SPD) Expert: Carol Stock Kranowitz, M.A. As a music, movement and drama teacher for 25 years, Carol Stock Kranowitz observed many out-of-sync preschoolers. To help them become more competent in their work and play, she began to study sensory integration (“SI”) theory. She learned to help identify their needs and to steer them into early intervention. In her workshops and writings, she explains to parents, educators, and other early childhood professionals how sensory processing issues play out – and provides practical and enjoyable techniques for addressing sensory issues at home and school.

In addition to The Out-of-Sync Child: Recognizing and Coping with Sensory Processing Disorder, revised (Perigee, 2005), Carol is the author of its sequel, The Out-of-Sync Child Has Fun: Activities for Kids with Sensory Integration Dysfunction (Perigee, 2003).

What inspired your interest in Sensory Processing Disorder?

Carol Stock Kranowitz: I wondered why some of my preschool students were not “doin’ what comes naturally.” They were bright and healthy, yet they responded in unusual ways to classmates, teachers, objects, and ordinary nursery school activities. Some children avoided altogether the experiences that their schoolmates enjoyed, others dove pell-mell into activities without an ounce of precaution, and still others tried to participate but did so clumsily. Were these out-of-sync kids behaving inappropriately on purpose? Of course not! No child seeks disapproval of his “significant olders.” Every child wants to learn; every child wants to play and have friends. Something else was going on that made it so difficult for them to succeed in their occupation of childhood. Until I learned about Sensory Processing Disorder, I could not find a pattern in these children. The only common thread — and this is what troubled me the most — was their disconnectedness. They lacked a sense of belonging. Whether their modus operandi was hostility, aggression, anger, frustration, sadness, bewilderment, tuning-out, whining, silliness, or wildly inappropriate gusto, they all seemed to sense that they weren’t like the other kids. Their behavior suggested that they didn’t know how to behave. There had to be an explanation, and I had to find it.

What is the prevalence of SPD?

Carol Stock Kranowitz: Estimates range from 5% to 20% of people of all ages all over the world.

What do you find to be the most common sensory problems among children?

Carol Stock Kranowitz:

Children with Sensory Processing Disorder exhibit unusual responses to touch and/or movement experiences. They also may have atypical responses to other sensations – sight, hearing, smell and taste.

A common difficulty is Sensory Modulation Disorder. This category of SPD makes it challenging to regulate sensations coming into the central nervous system from the child’s own body and the world around him. If children are over-responsive to touch sensations, these “sensory avoiders” will shy away from touching and being touched. They are on alert and ever vigilant to protect themselves from real or imagined hazards in a scary and confusing world. They will avoid messy play, physical contact with other children and adults (except for a select few, such as parents or a favored babysitter, to whom they will cling), pets, certain textures of fabric, many foods, bumpy sock seams, etc. If they are under-responsive to touch sensations, they may be “sensory disregarders” and not notice what they are touching or what touches them. They are not defensive enough and are more likely to have trouble protecting themselves. They may not feel pain or be aware of the world around them. Or, if they are “sensor cravers,” they’ll excessively seek touching and being touched. They will be fingerpainting their arms, stuffing too much food into their mouths, shouting indoors, turning up the volume, and bumping and crashing into people and furniture. If children with a Sensory Modulation problem are over-responsive to movement experiences, their feet will never leave the ground. They will shun playground equipment and object to riding in the car or elevator. They may refuse to be picked up. If they are under-responsive, they may not know when they are falling or that they have been on the swing for too long. If they crave intense movement, they may seem always to be in upside-down positions, swinging on the tire swing for long periods, and on-the-go constantly — jumping, bouncing, rocking and swaying.

Some children with SPD have Sensory Discrimination Disorder, which is difficulty discriminating among and between sensations. I call them “sensory jumblers.” Is the water hot or cold? Is the sand hard or soft? Does the button go into this buttonhole or that one? Is the bathroom is this direction or the other?

Some children have Sensory-Based Motor Disorder. One type is Postural Disorder, causing problems in getting into new positions and maintaining them. These are “sensory slumpers.” They struggle to go through obstacle courses. They slouch. They fall off the swing. They need support to hold themselves up, because defying gravity seems to be too much for them. And some children, “sensory fumblers,” have another type of Sensory-Based Motor Disorder, called Dyspraxia, which is difficulty in coming up with an idea of an action they want to take, such as getting on and riding a scooter; doing the motor planning necessary; and executing their motor plan.

It is important to note that many children are over-responsive to sensations, covering their ears when a truck rattles by, or pinching their nostrils to avoid smelling an old banana. Many children are undersensitive, perhaps liking spicy pizza and fireworks more than others do. Many children have occasional problems discriminating between and among sensations. Many children have moments of slumping in their chairs and clumsiness when they tackle a novel and complex sequence of actions. We wouldn’t necessarily say that these kids with occasional sensory difficulties have Sensory Processing Disorder. It is unusual, persistent reactions to touch and movement, making life very challenging, indeed, that suggest SPD.

If parents suspect that their child may have a sensory processing disorder, what do you suggest they do?

Carol Stock Kranowitz:

Be a detective! Keep notes on your child’s atypical behavior. Does his reaction to a sensory stimulus occur with frequency, intensity and duration? For instance, does the child have a heck of a time calming down after getting a splinter in his finger or being accidentally knocked down?

• Ask yourself the “WH” questions, i.e., When did it happen? Where? Who was involved? What happened or what was said? How did your child respond? After taking notes for a while, you may be able to see the pattern and find the answer to the trickier question of “Why did it happen?”

• Share your observations about your child with his teacher. The teacher may have noticed similar challenges. Collaborating with all adults who care for your child will make life easier for everyone.

• Find an occupational therapist certified to provide Sensory Integration treatment. This is referred to as “OT/SI.” (Only about 20% of occupational therapists are.) For a list of certified therapists, contact

Two highlights of the Activity Table include the following 2 activities: wooden beads which move in a spiral pattern along brightly colored coated wires and wooden shapes which move in a spiral slot and both provide baby with an opportunity to work on his eye tracking and eye hand coordination skills whilst having lots of fun. There is a spinning flower which provides auditory stimulation when it is turned and can also magnetically hold the magnetic wooden blocks. We are very excited to bring you our first recipe book feature today! A few months ago, we headed to San Francisco, California and stumbled upon a restaurant by the name of “Cafe Gratitude”.


We hadn’t heard of it before, but since we were famished and have a spirit of adventure, we thought lets give it a try. The evening was upon us and we needed a place to rest our legs and recharge our energy. We hadn’t known at the time that we had stumbled upon the perfect place to do just this! We were immediately struck with the interior decor and warm and friendly staff and sensed that we were in for a real treat that night. A waiter soon arrived and introduced us to the restaurant, concept and menu. He explained that the menu list was in the form of affirmations, so when we order we would order “the affirmation”. To view the menu, please click here. Cafe Gratitude is a raw foods, organic restaurant. All the food served here is vegan and the restaurant attracts a wide variety of culinary taste buds daily. We were new to the “raw foods movement” and were very excited to taste the food. We were very impressed with the wonderful flavors and textures of the food and vowed to return!

We later found out that the restaurant would be publishing a selection of the recipes and we knew straightaway that we were interested in featuring the recipe book. We would like to thank North Atlantic Books for sending us a review copy as well as the staff at Cafe Gratitude for their culinary support and guidance! Raw food recipes in general call for a dehydrator, but we never had one available and used our oven on the lowest setting instead. The dishes would have been considerably easier to prepare should we have purchased one. Some of the dessert ingredients were particularly hard to find (Irish Moss which is listed as a key ingredient), so perhaps in future editions, the authors can suggest a suitable replacement which may be more readily available. Almost every recipe requires that the ingredients are soaked for several hours and a good quality mixer such as a Vita-Mix is essential for blending the soaked ingredients!


We were eager to get started and decided to prepare the following 6 dishes and lined up a team of eager taste testers:

I am Relishing (Buckwheat Crackers)

One really needs to have a dehydrator to make these crackers! Ours took quite a while to set up in the oven, but the end result was worth it. We ended up with a delicious tasting vegetable cracker! Yum.

I am Cheerful (Sun Burgers)

The entire family enjoyed these Burgers which tasted fresh and flavorful.


Almond or Hemp Seed Pesto over Quinoa

This pesto was simple and fast to prepare and served over Quinoa, became the new favorite way to eat Quinoa.
I am Youthful (Almond Milk)
This was our first attempt at making Almond Milk. It was more time consuming then we thought that it would be, but well worth the effort. The taste of fresh almond milk is simply heavenly and nothing like the store bought variety! We must invest in nut milk bags plus a good mixer which will save us time!
Vanilla Hazelnut Ice Cream

This ice-cream is truly delicious and refreshing and the rich creamy flavor won over our panel.

I am Thrilled (Cinnamon Rolls)

This recipe received the top award from our panel of taste testers and we were thrilled that we were able to use the oven successfully to prepare them! The taste and texture of these cinnamon buns will leave you satisfied and you can rest assured knowing that they are heart healthy.

What inspired you to produce this movie and what were you goals?

Debby Takikawa:
For many years now I have been committed to offering information to families about birth, babies and early childhood parenting. Personally, I had some very sad moments when I realized how unaware I was as a young parent raising my children, and how little information I had to help me understand them, and how much misinformation I had about the nature of infants and young children. I looked back on all the things that I did that were not in the best interest of my children, and I decided that I would like to support families to have the information they need, information I wish I would have had, to make good parenting decisions. In the year 2000 I opened Beginnings Inc., an NPO with a threefold mission. The clinic was for working with babies and their families who felt they had attachment questions, challenges or issues. The clinic was also a teaching program for MFT (Marriage and Family Therapist) interns who wished to explore the therapeutic process with babies in order to more fully understand adult issues that originated during the developmental years. The third part of the mission was public outreach. It was through the public outreach part of the organization that I decided making a short little educational video would be a good idea. I placed my idea before the board of directors, who were just as unsuspecting as I was about the enormity of the way the project would unfold!My intention was to be able to show the video to people who were interested in the clinic, but didn’t understand what we did or why a baby would be able to benefit from our work. The video turned out to be a real documentary film. It went to film festivals and won awards! By now hundreds of thousands of people have seen it. I ended up showing this film all over the country, and as a result, I have sold over 10,000 copies, and many of those copies have been used over and over in parenting and birth classes. I receive letters nearly every day from practitioners who say that it helps families to understand their babies more than anything else they have tried. It opens the door to conversation and affirms instinctual healthy parenting impulses that have been suppressed by years of cultural beliefs that babies were not aware and are not able to learn or feel or communicate. It has been an exciting process for me because I think the film works for people who do not know that their babies are already sensitive people with feelings and sensations and meaningful thoughts and experiences of their own. So What Babies Want is a great tool for outreach. There is a huge emerging field of birth practitioners who are reaching out to the public, and this film is extremely helpful for that purpose. When I made the film, I had no idea that it would take me so far!

This DVD is a revolutionary documentary which examines a question that no-one asks “What Babies Want”. Narrated by Noah Wyle and featuring world renowned experts who examine and explore this question.

We learn in this DVD that babies are indeed conscious from birth and are not simply “blobs”. We were particularly touched by Sobonfu Some, who’s name means “Keeper of the Rituals” who shares the traditions of her tribe who honor and celebrate
children and community.

Sebonfu teaches us that in her tribe, they appreciate women and the gift that they are about to bring into the world (unborn child). One of the most touching scenes in the documentary takes us into the forest where Sobonfu leads a group of women who
sing and welcome and honor a group of pregnant women with beautiful traditional songs. Researches share that a womens emotional state during pregnancy determines the shape and structure of the babies brain. How an infant bonds is explored in depth and this fascinating documentary suggests that an infant can have a 3 month delay in development if an infant does not bond successfully with his parents immediately following birth. Of particular importance is the chapter which examines current hospital protocols which separate healthy full term babies from their parents. We learn that if babies are separated from their parents at birth, they will bond to machines and will lose the opportunity to develop trust. Well known Pediatrician Jay Gordon emphasizes that “healthy full term babies stabilize much much better in moms arms than away from her”.Until recently, many health professionals thought that babies did not experience any pain and thus handled infants roughly. We now understand that this is completely untrue and the manner in which a baby is welcomed into the world is of key importance to his future as we track our experience of life from birth to death.

Baby Expo and International Motherbaby Film Festival Review

We have just returned from the Gentle Birth Baby Expo and International Motherbaby Film Festival in Portland, Oregon. Highlights included the interesting array of classes that were offered free to the public, a fabulous Babywearing Fashion Show which was produced by the NW Babywearers and thought provoking and educational films that were screened at the Motherbaby Film Festival. We walked around the small expo itself and had the opportunity to speak with a group of Certified Nurse Midwives (CNM) from the OHSU Midwifery Group. We were pleased to learn that they offer a waterbirth option which is rare to find within a hospital setting. If you reside within the Portland Metro Area and are interested in having a natural birth within a hospital setting, you may wish to find out more about the practice. We were excited to see that one of the exhibitors were: EARTH MAMA ANGEL BABY® who we reviewed back in June 2007. They had set up a beautiful display of their product line!

We arrived at the Oregon Convention Center in Portland, Oregon and were impressed with the building and the grand scale of it. The signs to the Gentle Birth Baby Expo were clear and pointed us in the right direction. We were warmly welcomed and glanced at the schedule of the events for the day. All the events at the Baby Expo were made available free to the public, which is really exceptional!

We decided to begin our experience by attending a class. Many interesting classes were held simultaneously and it was difficult to select which classes to attend. The first class that we attended was “Stomp! Squat! Positions for Labor and Birth” by a local Doula. With the use of a doll, a pelvis and a pipe and a sweater, the doula proceeded to educate the audience about why lying flat on your back during labor is not ergonomically correct and doesn’t aid in the birth process. We were taught a variety of positions which women can assume during labor, which help the baby to move downward and rotate through the pelvis to allow for as easier birth. The use of a birth ball was emphasized!

Following this class, we attended a talk by Carol Penn, DO in the Bold Red Tent, who led us in a guided meditation session.

We headed to a Babywearing Fashion Show which was produced by the N. W. Babywearers group. We thank them for allowing us to take these pictures! (to follow).

Following the Fashion Show, we stayed for a class titled: “Baby Carrier FUNdamentals: Overview of the Basic Carriers & Their Uses which was led by 2 moms: Heidi Donnelly & Dacia Houser.

We learned about the variety of different carriers that are on the market, including many made locally in the Portland, Oregon area. The speakers were clearly knowledgeable and shared a passion for babywearing that was contagious!




We then attended a though provoking class entitled: “Autism & Environmental Influences” which was taught by Dr. John A. Green III, MD. Dr. Green specialized in working with families who have a child on the autistic spectrum and wish to conceive again. Dr. Green comments that parents should be concerned that the rate of allergies, asthma, learning disorders, depression, diabetes etc are on the increase and believes that our toxic environments are playing a key role in this. He explains that babies/children are more susceptible to toxins and that we should make every effort to reduce our exposure by eating organic produce, avoiding teflon, food additives, using less plastics, reducing our consumption of animal fats, adding omega 3 fatty acids to our diet, eating a nutrient dense diet and living in an eco-friendly home (zero voc paints, avoiding carpets etc). Dr. Green offered guidelines for selecting items for your baby’s environment (cotton diapers, edible toys, natural bedding etc). Dr. Green offered parents very practical advice about how to reduce the chemicals and toxins in their homes and given his extensive experience and knowledge working with families who have been exposed to toxins, we feel that this class was the most informative.

We dashed out of the conference to have a wonderful serene Indian dinner at the Tao of Tea and returned in time to view a private screening of the New Unfinished film: Pregnant in America which was held upstairs in the beautiful Ballroom. Gentle Birth Conference Attendees, attendees of the Baby Expo and the general public filled the Ballroom. Barbara Harper, R.N, founder of Waterbirth International introduced the film. The documentary explored one pregnant couples quest to seek out their birth options which led them to several European countries in the process. The film featured many well known experts in the field of infant and maternal health and was certainly thought provoking.

We attended the screening of the beautifully directed film “What Babies Want” which we will be reviewing next. Afterwards, there was a panel discussion with the film’s director, Debby Takikawa and two experts who were featured in the film, David Chamberlain and Sally Clark. The audience asked questions around their or their children’s birth experiences and the panel addressed each question with wisdom and sensitivity. We have certainly been inspired to find out the story of our birth.

Today we decided to spend the day previewing films and headed directly to the Motherbaby Film Festival. During the morning, the film, Breastfeeding Basics was shown, and we were pleased to see that Mother of 7 had co-sponsored the Motherbaby Film Festival. We recently posted 2 articles about Stephanie Neurohr, the exceptional Mother of 7:

We were introduced to a remarkable film called “Breast is Best” from Oslo Norway which was responsible to increasing the breastfeeding rate in Norway from close to zero to almost 100%. Under the direction of the Norwegian Government and in an effort to increase the rate of breastfeeding, this film was played throughout the country of Norway in the cinemas, on television and during public events. The effort was an enormous success and we can only hope that other countries will follow suit. The film teachers mothers how to successfully breastfeed and how to overcome common breastfeeding problems.

Following this film, we were introduced to 2 outstanding individuals, Kittie Frantz, R.N., CPNP-PC and Dr. Chistrina Smillie (Pediatrician). We then proceeded to view the film “Baby-Led Breastfeeding” which explained that full term healthy babies will naturally crawl up to mom’s breast and breastfeed on his own! A baby will naturally assume the correct angel etc when he/she is allowed to take the lead. Mom and dad can assist as needed, but should let their baby lead. The expert panel emphasized that moms and dads need to learn to recognize their babies cues and respect when baby tells them that she has had enough milk. Forcing babies to feed when they are full can potentially increase their chance of becoming obese in later life. We were taught that swaddling a baby can switch off his hunger sensation and can potentially interfere with his desire to eat. The latest research indicates that swaddling should be reserved for very fussy infants only and used as a last resort. When possible, hold your baby when you swaddling/wear them vs leaving them alone, swaddled in a crib.

Overall, the 3 day Baby Expo and Motherbaby Film Festival was exciting and educational and a wonderful resource for parents. We hope to return next year!
